Detailed information on any previous or current treatment
If you have used any creams to treat a rash, please bring a list of the names of the creams, or better yet bring the creams themselves. - Insurance card

Referral
Most insurances require a specific referral from your primary care physician. Sometimes this is sent directly from the primary care office, but if you have a copy of the referral or the referral number, bring it with you! -
Co-payment
Most insurances require a co-pay due at the time of the visit. We accept personal checks or cash; we also accept MasterCard, Visa, and Discover. -
Translator, if you don't speak English
If you are unable to speak English, please bring with you a person who can translate for you. -
New patient intake forms
For a more efficient first visit, feel free to print and fill out our Forms for new patients before you arrive.
Canceling or changing an appointment
- Out of courtesy to us and to other patients, please let us know at least 24 hours in advance if you wish to cancel or change your appointment. If you forget or are unable to call us during business hours, you are welcome to leave us a voicemail message. If you do not show for a scheduled appointment you may be charged a nominal fee that will not be covered by insurance. We will waive this fee for the first occurrence, but if it happens repeatedly you will be billed.
